We know that each patient is different and has specific needs, so we strive to provide you with personal and individualized care. If you bring your cell phone to your appointment, be certain to turn it off prior to being called back into the clinic. How to prepare for your new patient visit:
ReferralsIf your insurance requires a referral, you are responsible for obtaining the referral and seeing it reaches our office before your appointment. We also ask that you have your physician send any documentation, test results, etc. that would provide our physicians with pertinent information to help with your diagnosis and treatment. Additionally, this will help prevent unnecessary duplication of tests. InsuranceWe accept most major insurance plans. Please check with your insurance company to see that our physicians are listed on your plan. Please note that we collect insurance co-pays and deductibles at the time of your visit. Self-paySelf-pay patients will be required to make a payment of $80 toward the cost of their visit. Payment arrangements can be made for additional balances if necessary. Delinquent accountsPatients with seriously delinquent accounts will be required to make their accounts current before being seen by a provider. |
